12 MONTH TRANSITIONAL PROGRAMME
The details and length of the client’s transitional programme are
formulated after a meeting between the client and their therapist. During
that meeting the client identifies their goals and what they would like to
achieve in various time frames. The therapist assesses the feasibility of
the client’s goals and the aftercare programme is tailored on that basis.
The client’s progress will be monitored and assessed by our therapeutic
team consistently throughout their transitional programme. Similarly, they
will receive a thorough assessment at the end of their programme and they
will be given the option of continued support if desired by them or
recommended by their therapist.
The following is an example of a 12 month aftercare programme in Byron
Bay, designed for a past client:
Phase One – Month 1: Intensive Transitional Programme
The first phase of your aftercare programme will be intensive in order to
ensure your needs are assessed and addressed effectively and to ensure
you receive the support you will require to begin living a positive lifestyle.
This part of your programme will incorporate the following:
Accommodation – you will live in supported accommodation with recovered
addicts who have a substantial amount of time in recovery, providing you
with the consistent support and guidance needed in your home environment.
Medical Assessment and Consultations – you will be assessed by a GP
upon arrival and you will receive follow up consultations as required, to
ensure any health issues are addressed.
Psychotherapy – we will schedule three psychotherapy sessions for you
each week. We recommend regular and frequent sessions in the first month
and continuing for the following few months. This means that the therapist is
able to attain a comprehensive assessment of your needs and you can
develop a deeper therapeutic relationship with your therapist in a short
space of time.
Physiotherapy – we will schedule a physiotherapy assessment and follow-up
sessions if required.
Bodywork – at The Sanctuary, we offer a unique combination of Eastern and
Western therapies to ensure a holistic approach. We recommend regular
bodywork sessions including yoga and meditation, shiatsu/qi gong,
acupuncture and massage, one to three times per week (for each modality
used) for the first month. We will schedule these sessions each week,
however, we will be flexible and responsive to your evolving needs and
wishes.
Personal Fitness – we will schedule one to two sessions with our personal
fitness instructor each week in the first month. We will also incorporate
alternative fitness activities, such as martial arts, surfing and dancing,
depending upon your personal preferences.
Naturopathy – our naturopath will meet with you to assess your dietary
requirements and prescribe naturopathic supplements in order to bring you
to optimum health in the shortest possible time. Our naturopath will work in
conjunction with the GP in order to ensure an effective, collaborative
approach. Follow-up consultations will be scheduled as required.
Nutrition and Cooking Classes – one of our highly experienced chefs will
give you nutrition and cooking classes once a week so you can learn to
prepare healthy and enjoyable meals on your own.
12 Step Fellowship Meetings – we will organise your attendance at 12 step
NA and/or AA meetings with a support buddy, either daily or less often,
depending upon your preference as to frequency
Phase Two – Months 2-4: Less Intensive Transitional Programme
This phase of your aftercare programme will be less intensive to encourage
you to become more self-empowered and independent. However, you will
continue to be closely monitored to ensure you stay on track and receive the
encouragement and support you require.
All aspects of the programme in Phase One will continue, however, the
frequency of some of the sessions will be reduced. For example, bodywork
sessions will be reduced to twice a week (for each modality used).
12 Step Fellowship Meetings will continue as in Phase One.
Volunteer work – we will encourage you to give back to the community in the
form of volunteering at local pet shelters, environmental organisations or
aged care facilities. We find volunteer work is an effective means of
extending a person’s perspective beyond their own experience and an often
rewarding and fulfilling activity to constructively occupy some of their time. It
is also a highly effective means for a person to meet people and develop a
wider social network.
Adult Education – if appropriate and providing you feel ready, we will
encourage you to take part in a course of interest to you, such as learning a
language, doing a computer course or a course in small business
management. There is an extensive list of interesting courses available
through our Adult Community Education programme. Again, this will further
occupy your time constructively as well as allowing you the opportunity to
further develop your social network. In addition, it will encourage you to start
thinking about and aspiring towards a fulfilling career path.
Phase Three – Months 5-9: Semi-independent Transitional Programme
This phase of your programme will be less formally structured and you will
be encouraged to begin taking control of your appointments and
commitments. Again, this will depend upon where you are at and how you
are feeling at this point and we will only act in accordance with the
recommendations of your therapist.
All aspects of the programme in Phase Two will continue, however, the
direction of your programme will be adjusted according to your evolved
needs. Psychotherapy sessions will remain the same, however, in relation
to bodywork, you will be encouraged to attend group classes rather than
one-on-one sessions. You will also be encouraged to attend a group fitness
activity, such as a martial arts class or a group sport, depending upon your
preference.
12 Step Fellowship Meetings will continue as in Phases One and Two.
Adult Education – if you haven't already embarked on a course in Phase
Two, we will encourage you to take part in a course of interest to you, as
outlined above. If you have already completed one course and you would
like to enrol in a further course, we will assist you in doing so.
Phase Four – Months 10-12: Independent Transitional Programme
This final phase will be even less formally structured, however, again, it will
be within the recommendations of your therapist and you will know you are
still being monitored and supported by The Sanctuary, albeit to a far lesser
degree. We will be encouraging you to start thinking about potential career
paths, financial responsibility and independence.
All aspects of the programme in Phase Three will continue.
12 Step Fellowship Meetings will continue as in Phases One, Two and
Three.
Adult Education will continue – we will encourage you to take part in further
courses if the previous course/s has/have been completed, to further explore
and develop your interests.
Careers Counselling – we will make an appointment for you to see a careers
counsellor to encourage you to think about a career path that is viable and of
interest to you. Depending upon the outcome of that session, we will assist
you in pursuing either study towards your chosen career path or we will
assist you in actively seeking employment in that field.
Financial Advice – we will schedule you to see a financial adviser to provide
you with the guidance required to manage your finances responsibly and
constructively. This will further enhance your independence and ensure you
are on track before you complete the transitional programme.
Financial Planning and Budgeting –
Throughout your transitional programme at The Sanctuary you will have
weekly meetings with one of the therapists to discuss anticipated weekly
expenses and to set a weekly budget. This process will have the benefit of
teaching you financial responsibility and help you to learn to live within your
means. It will also give you a regular structure of planning your week and
deciding what you are going to do and working out how you are going to
facilitate those plans.
